Member Plan Options

Our two plan options, Standard and Enhanced, require members to complete health screenings, create health goals, and have a physical each year. The same essential health benefits are covered in both plans.

Option A: Enhanced Plan
With lower premiums and copays, this plan has an additional requirement: that members participate in our Continuous Health Improvement (CHI) program, which includes a health coach partnership and taking health education courses.

Option B: Standard Plan
This plan includes the same coverage as our Enhanced Plan, but with higher copays and premiums, and without the CHI Program requirement for members.

Because our financing arrangement combines resources from community stakeholders with a vested interest in community health – including care providers, hospitals, and businesses members only pay 30% of the premiums.

General Eligibility Guidelines

  • Individuals must live or work in the Greater Muskegon area.
  • Individuals must be uninsured and not eligible to receive coverage through any other source including Medicaid, Medicare, or employer-sponsored coverage through themselves, their spouse, or their parents, with the exception of coverage available through the health exchange.
  • Individuals must have a projected annual household income between 133% to 300% of the Federal Poverty Level.
  • Individuals must be US Citizens or have been a Lawful Permanent Resident for a minimum of 5 years immediately preceding application for the Access C3 Health Plan.
