We believe that success starts with relationships, and we understand that getting healthy is more than just eating well and exercising. This is why we get to know more about people than just their physical health.
Through one-on-one conversations and assessments, we learn about health risks and concerns — and we find out what other things in your life may make it difficult for you to get healthy and stay healthy.
Every year, we will work with you to set small but realistic health goals for yourself.
While not a comprehensive list, the following essential benefits are included in both our Standard and Enhanced Plans.
We offer two plan options, which provide the same health benefits coverage but have different copays and monthly premiums.
The difference between these plans is that our Enhanced Plan requires members to enroll in our Continuous Health Improvement (CHI) program to take advantage of cost-savings.
Because our health plan is supported by funds and resources from community, members only pay 30% of the premiums.
for both Enhanced and Standard Plans
You will begin this program by working with our health coach to create your own Comprehensive Health Improvement Plan, which includes specific yet realistic health goals. Through a one-on-one relationship the coach, you’ll get the support you need to reach your goals, and you will have the opportunity to ask questions or raise concerns about your health.
The CHI program also requires that you attend health education courses each year. Below is a list of our current courses. Please contact us for details about when they are offered.
Health is more than just eating well and exercising, it also includes understanding the struggles and barriers that hinder our success at meeting goals. This is why we have integrated the Domains of Health within everyone’s wellness plan.
