The C3 Plan (Choice, Challange & Change) was developed to encourage personal accountability by engaging employers to provide healthy work sites and employees to embrace healthier everyday choices. The key to turning things around, by improving health and keeping health costs down, lies within each of us. To support personal accountability the C3 plan includes a Health Risk Appraisal and face-to-face health advising to collect information to identify health risk factors, provide individualized feedback/action plan, and links members to programs that promote health improvement and/or prevent disease. We encourage members to make a personal investment in their health and take the actions necessary to improve health. The services and classes are offered at no charge (unless otherwise stated) to the employees and their families. We hope all employers and employees embrace the opportunity the C3 plan provides.

C3 Overview:
C3 members include the employee and all enrolled family members
The C3 plan is ongoing and requirements must be met ANNUALLY. To keep the C3 plan you must complete the requirements every year.
Your C3 “Anniversary” is the date you enrolled in the C3 Plan. Contact Access Health if you don’t know what your date is.
The entire family unit must be assigned the same plan, either C3 or Standard
The entire family is considered a “unit” and every person is required to complete the requirements. If the requirements are not met by all of the enrolled members, the entire family would be transferred to the Standard Plan. Family members age 19 and above are required to complete the requirements.
Members and Businesses are encouraged to call Access Health at 231-728-5180 x100 to obtain a status of employee participation or to schedule requirements
As a courtesy, reminder notices may be mailed to the employee and the business. The employee is responsible for scheduling and completing the requirements. Access Health does not call the members to schedule their appointments.
Deadlines: The Health Risk Appraisal should be submitted upon enrollment or within 3 months of your anniversary date. The Health Advising appointment must be completed within 6 months of the anniversary date. The classes must be completed within 10 months of the anniversary date.
Note: Standard Plan members are required to take classes if they have a chronic disease or illness as part of a “protocol”. (Examples, Tobacco Cessation, Asthma or Diabetic Education)
Cost:
The C3 Plan costs $46 (each adult) and $29 (each child) for both the business and employee. The benefit is, lower monthly cost for the employee and employer and lower co-payments for services.
The Standard Plan costs $60 (each adult) and $36 (each child) for the business and employee.
Refer to the Member Handbook for co-pays. The Standard Plan member pays significantly more for the monthly cost and for co-pays.
“Grandfathered” Sole Proprietors/Owners Only: C3 Rates: $106 each adult, $58 each child and Standard Rates: $140 each adult, $72 each child
Annual Requirements (Each family member):
Complete a Health Assessment and Lifestyle Questionnaire- Due upon enrollment and then annually within 3 months of your Anniversary/C3 Enrollment Date
Attend Health Advising Appointment where the member meets with a health advisor and creates a personalized health action plan- Due within 6 months of enrolling and within 6 months of your Anniversary/C3 Start date annually.
Attend two health classes. If the member smokes, one of the classes must be “Tobacco Free” offered at the Muskegon County Health Department- Due within 10 months of your anniversary date annually.

NUTRITION:
Healthy Eating on a Budget: Learn ways to incorporate healthy foods into your daily eating plan even when the food budget is limited.
Healthy Eating While Eating Out: For many people eating out is a necessary part of daily life. Learn how to select healthier choices from the menus of area restaurants.
Healthy Weight Management: Learn how to develop a healthy eating plan for you that is not a diet and will lead to gradual and permanent weight loss.
Building Your Pyramid: Is your goal to eat healthier? Learn how the Food Pyramid helps you. We invite you to test your knowledge on what you should be eating.
Aim for a Healthy Weight: Develop a healthy eating plan for you, not a diet, that will lead to gradual and permanent weight loss.
Taste of the Season: Access Health, in partnership with Muskegon MSU Extension, hosts seasonal cooking classes. Members who particpate will gain hands on experience with new, healthy recipes using locally grown produce. Learn money saving recipes, while eating healthy and supporting local farmers.
PHYSICAL FITNESS:
Introduction to Tai Chi: Learn the history and philosophy of Tai Chi. Discover the benefits for those with and without chronic health conditions.
Random Acts of Fitness: Learn the importance of movement, activity, and nutrition to enhance the body and mind on a daily basis.
Stretch for Health: Learn how to incorporate stretching in your daily routine and how to avoid injury caused by exercising.
Band on the Run: This class demonstrates a total body workout using only one compact, affordable, totally mobile piece of home gym equipment. Come prepared to participate.
HEALTH ENHANCEMENT:
Men's Health: This class is taught from the male perspective and can help men of any age get a better understanding of their individual health needs.
Stress Management 101: This class provides an overview of how stress has a significant impact on your emotional and physical well-being. Learn techniques that can be used to decrease stress.
Stress Management 201: Learn in-depth how stress contributes to many chronic diseases. This 3 hour class series will help you learn ways to handle the stress and understand the long term impact of stress on our bodies. Stress 101 is a prerequisite.
Tobacco Cessation Class - “Smoke Free”: This is a class offered by Public Health- Muskegon and Access Health staff to provide education about tobacco cessation and the tools to quit. This class is offered monthly starting the 1st Tuesday of every month for 4 consecutive weeks at Muskegon Public Health and during the day at the Access Health office (days/times vary by month) . Call 231- 728-5180 to register. This class is a yearly requirement if you are a smoker. Access Health has provided grant funding so Access Health members and their family members do not pay the class fee.
Emotional Eating: Many people find that nutrition and exercise are not enough to maintain a healthy weight. The missing link is often related to emptional eating habits. This introductory class will help you determine if emotional eating is getting in your way of maintaining your healthy weight, and will offer strategies to address this common experience.
Patient Empowerment: Who is the most important person on your health care team? You! In this class you learn what it means to have quality health care and what your part is in improving your health outcomes.
Ear Acupuncture: Beneficial for those who are tobacco users, have weight issues, stress and anxiety. Held on Friday at 3:30 PM at the Access Health office. This is performed by health professionals trained in ear acupuncture.
CHRONIC DISEASE MANAGEMENT CLASSES:
Asthma Education: This class teaches participants the tools to manage their asthma. This class uses the National Asthma Guidelines and is taught by a Nationally Certified Asthma Educator.
Healthy Heart Seminar: This seminar is for those who want to improve lifestyle starting with their heart. Facts and tips on cholesterol, high blood pressure, and exercise.
Diabetes Management:This class is designed for persons newly diagnosed with diabetes and their support persons. This class may assist those who were previously diagnosed, but it has been poorly controlled or if you have questions about how to manage diabetes. This is a series of four classes; Know your Numbers, Know Your Nutrition, Know Your Healthy Steps and Know Your Options.
PATH (Personal Action Towards Health) Healthy Living Workshop - Stanford Medical Research: This course is for individuals who have diabetes, lung disease, health disease, chronic pain or other chronic conditions. The class covers symptom management, nutrition, exercise, communication skills, relaxation techniques, goal setting and problem solving. Class sessions are 2.5 hours and meet for six weeks.
Effective Coping Mechanisms: (Prerequisite SM201) Explre the "ineffective coping mechanisms" used when we are challenged. Learn how to change ineffective to effecting coping strategies. This is an interactive class for those who have completed SM 201 already. If you want a class you can attend and sit quietly, this class is not for you.
